Mode and Effects Analysis) ist eine Methode, mit der ein Team m\u00f6gliche Fehler eines Produkts oder Prozesses vorab sucht. F\u00fcr jede Fehlerkette werden Bedeutung, Auftreten und Entdeckung bewertet, daraus folgen Ma\u00dfnahmen. Ein Team beantwortet sie, bevor das Produkt in Serie geht. Deshalb gilt die FMEA als vorbeugende Methode. F\u00fcr die Analyse eines Fehlers, der schon beim Kunden aufgetreten ist, gibt es andere Werkzeuge wie den <a href=\"\/blog\/allgemein\/8d-report\/\">8D-Report<\/a>.<\/p>\n<p>Im Kern steht die Fehlerkette. Eine Ursache f\u00fchrt zu einer Fehlerart, die Fehlerart zu einer Fehlerfolge. Ein Beispiel aus der Montage: Der Schrauber hat sich verstellt (Ursache), das Drehmoment ist zu niedrig (Fehlerart), der Deckel lockert sich im Einsatz (Folge).<\/p>\n<p>Eine fr\u00fche dokumentierte Grundlage ist die US-Milit\u00e4rnorm MIL-P-1629 von 1949. \u00dcber Raumfahrt und Automobilindustrie hat sich die Methode verbreitet. Man untersucht zuerst ein ganzes System mit seinen Schnittstellen und geht dann in Baugruppen und Teile. FMEA-MSR erg\u00e4nzt nach AIAG-VDA die DFMEA und hat eigene Bewertungskriterien. Vorhandene FMEAs \u00e4hnlicher Produkte sind ein guter Startpunkt.<\/li>\n<li><strong>Strukturanalyse:<\/strong> Das Betrachtungsobjekt in Elemente zerlegen. Bei der PFMEA sind das die Prozessschritte und ihre Einflussfaktoren wie Mensch, Maschine, Material und Umfeld.<\/li>\n<li><strong>Funktionsanalyse:<\/strong> Jedem Element seine Funktionen und Anforderungen zuordnen, etwa \u201eDeckel mit vorgegebenem Drehmoment verschrauben\u201c.<\/li>\n<li><strong>Fehleranalyse:<\/strong> F\u00fcr jede Funktion Fehlerfolge, Fehlerart und Fehlerursache bestimmen und verkn\u00fcpfen.<\/li>\n<li><strong>Risikoanalyse:<\/strong> Bestehende Vermeidungs- und Entdeckungsma\u00dfnahmen eintragen, Bedeutung, Auftreten und Entdeckung bewerten, Aufgabenpriorit\u00e4t bestimmen.<\/li>\n<li><strong>Optimierung:<\/strong> Ma\u00dfnahmen mit Verantwortlichem und Termin festlegen. Nach der Umsetzung wird neu bewertet und die Wirksamkeit nachgewiesen.<\/li>\n<li><strong>Ergebnisdokumentation:<\/strong> Ergebnisse, Restrisiken und Entscheidungen festhalten und an Management oder Kunden berichten.<\/li>\n<\/ol>\n<p>Fertig ist eine FMEA damit nicht. Sie wird bei Produkt- oder Prozess\u00e4nderungen, nach Reklamationen und bei neuen Erkenntnissen aktualisiert. Dazu kamen feste Grenzen wie \u201eab RPZ 100 handeln\u201c. In Fachforen berichten Anwender, dass Bewertungen dann so lange angepasst werden, bis der Wert unter der Grenze liegt (<a href=\"https:\/\/elsmar.com\/elsmarqualityforum\/threads\/when-to-take-action-if-an-fmea-rpn-number-is-high.43358\/page-2\" target=\"_blank\" rel=\"noopener\">Elsmar Cove Forum<\/a>). Schon das fr\u00fchere AIAG-Handbuch hat davon abgeraten, feste RPZ-Schwellen als Kriterium f\u00fcr Ma\u00dfnahmen zu nutzen.<\/p>\n<p>Das AIAG-VDA-Handbuch ersetzt die RPZ deshalb durch die <strong>Aufgabenpriorit\u00e4t (AP)<\/strong>. Sie ist keine Formel: Die Kombination aus B, A und E wird in einer Tabelle nachgeschlagen. Die Bedeutung wiegt dabei am st\u00e4rksten, danach das Auftreten, dann die Entdeckung. Das Ergebnis ist eine von drei Stufen:<\/p>\n<ul>\n<li><strong>Hoch (H):<\/strong> Das Team muss eine Ma\u00dfnahme festlegen oder dokumentiert begr\u00fcnden, warum die bestehenden Ma\u00dfnahmen ausreichen.<\/li>\n<li><strong>Mittel (M):<\/strong> Eine Ma\u00dfnahme oder Begr\u00fcndung sollte festgelegt werden.<\/li>\n<li><strong>Niedrig (L):<\/strong> Eine Ma\u00dfnahme kann festgelegt werden.<\/li>\n<\/ul>\n<p>Die AP sagt also, wie dringend gehandelt werden muss. Eine Risikoklasse ist sie nicht. Wer nicht nach AIAG-VDA arbeiten muss, kann weiter mit der RPZ arbeiten, sollte die Bedeutung aber getrennt betrachten und auf starre Grenzwerte verzichten. Bestehende FMEAs d\u00fcrfen laut VDA QMC bei Aktualisierungen im bisherigen Format bleiben, solange Kunde oder interne Regeln nichts anderes verlangen (<a href=\"https:\/\/vda-qmc.de\/wp-content\/uploads\/2024\/02\/Pressebericht-2020-QZ-Der-neue-FMEA-Standard.pdf\" target=\"_blank\" rel=\"noopener\">QZ 6\/2020<\/a>). Damit verschwinden genau die Risiken, die die Methode finden soll.<\/li>\n<li><strong>Falsch besetztes Team:<\/strong> Zuh\u00f6rer ohne Bezug zum Thema kosten Zeit, w\u00e4hrend Leute aus Fertigung, Instandhaltung oder Pr\u00fcfplanung fehlen.<\/li>\n<li>Ohne ge\u00fcbte Moderation verlieren sich Sitzungen in Detailfragen.
